Emergency Certificate

Requirement and process of applying for Emergency Certificate

• Please visit link : https://embassy.passportindia.gov.in 
• Select Flag of Cambodia (https://portal6.passportindia.gov.in/Online/index.html )
• Register yourself, then fill up online form for Emergency Certificate and submit it.
• Take print out and attach following documents (Police complaint in original, passport & visa copy, and copies of any two Indian documents (Aadhaar Card/ PAN Card/ Driving License/ Voter ID/Qualification Certificate from recognised Board/ University, along with originals)
• 5 passport size photographs with white background of size 2”x 2” (face should cover 70-75% of photograph)
• Submit all documents along with the Emergency Certificate application form to Embassy Consular Section in any working days between 0900 to 1100 hrs.

Registration of Birth Certificate

Documents required :

• Copy of duly appostiled/ attested Marriage Certificate of parents (original to carry to the Embassy counter)
• Photocopies of Passport of child’s parents
• Copy of Cambodian latest Visa issued to child’s parents
• Birth certificate attested by Ministry of Foreign Affairs of Cambodia
• Letter of employment in r/o either parent on Company’s letterhead
• Employment Contract containing: Name, Designation, Salary, Contract Start/ End date.
• Valid Work Permit of working parent
• Rent Agreement
• Bank Statement of working parent

[Note: original copies of personal documents should be accompanied with their originals to present at the Embassy counter for verification.]

Issue of Death Certificate

Documents required :

• Hospital Certificate/ Police Report
• Original Passport of the deceased

Attestation of documents

Documents required :

• Original and two photo copies of document
• Copy of passport (front and back pages)
• Copy of valid Cambodian Visa
• Original Passport
• Letter of employment on Company letterhead
• Employment Contract containing: Name, Designation, Salary, Contract Start/ End date.
• Work Permit
• House Rent/ Lease Agreement with English translation (if in Khmer)
• Bank Statement
• Plain application mentioning purpose of attestation.

Police Clearance Certificate

Documents required :

• Print out of submitted application online (for Indian nationals)
• An application in prescribed format for applying for Police Clearance Certificate (for non-Indian nationals)
• Self-attested copy of Passport
• valid Cambodian visa and endorsements, if any
• Original Passport
• Letter on Company letterhead
• Employment Contract containing : Name, Designation, Salary, Contract Start/ End date.
• Valid Work Permit
• House Rent/ Lease Agreement with English translation (if in Khmer)
• Any two Indian documents (Aadhaar Card/ PAN Card/ Driving License/ Voter ID/Qualification Certificate from recognised Board/ University, along with originals)
